Job Summary

We are seeking a skilled Bereavement Clinician to join our Hospice Home Care team. As a Bereavement Clinician, you will play a critical role in developing and implementing the Hospice Grief and Bereavement Program, ensuring that our patients and their families receive the highest quality care and support during their time of need.

Key Responsibilities
  • Assess grieving individuals and families, conducting individual and family sessions, and facilitating grief and bereavement support groups.
  • Develop goals and objectives in coordination with bereavement services, creating personalized bereavement plans of care for each identified client.
  • Collaborate with the Hospice Volunteer Coordinators to train, supervise, and evaluate Hospice bereavement volunteers.
  • Develop and implement annual Quality Improvement Plans for bereavement services, ensuring alignment with the overall Quality Improvement Plan for Hospice.
Requirements
  • Master's degree in Social Work, Counseling, or a related field.
  • One of the following credentials: Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Associate License: Social Worker Associate License (LSWAA) or (LSWAIC), Associate Mental Health Counselor (AMHC) or Associate Marriage and Family Therapist (AMFT).
  • Current Washington State driver's license and proof of insurability.
  • 2 years of experience in Social Work, Counseling, or a related field, preferably with experience working with people who are grieving or in a hospice program.
Desired Qualifications
  • 1 year of experience in program coordination in a healthcare setting.
  • Experience in program planning, development, implementation, and evaluation.
