We are seeking a skilled Health Coach Specialist to join our team at AndHealth. As a Health Coach Specialist, you will play a critical role in helping our patients reverse chronic disease.

Key Responsibilities:

  • Utilize evidenced-based health coaching interventions to improve patient engagement and outcomes.
  • Guide patients through the healthcare provider-created treatment plan, and partner with providers to provide safe, effective coaching and care to patients.
  • Collaborate with the Care Team to develop integrated treatment plans, and partner with medical providers to provide safe, effective treatment and coaching to patients.
  • Provide AndHealth integrated health coaching treatment approaches to a full patient panel, including strategies to target sleep, stress, nutrition, movement, and medication adherence.
  • Collect, interpret, and track patient-reported outcomes and wearable data, and use this data to inform individualized treatment plans, in collaboration with the patient's medical provider.
  • Support patients with personalized, medically necessary, and informed nutrition plans under the direction of a registered dietician to achieve less severity and frequency of migraine attacks and/or autoimmune flares.
  • Conduct virtual, synchronous treatment sessions with patients, to evaluate progress and adapt interventions to the patient's needs.
  • Engage patients in asynchronous and individualized coaching as needed using technology platforms.
  • Follow clinical workflows and processes using an EHR and other key tools to effectively document patient outreach and progress.
  • Actively contribute to process improvements by providing regular continuous feedback to internal stakeholders.
  • Assist with special projects as needed.

Requirements:

  • Bachelor's degree is preferred.
  • Completion of a health coach training program that qualifies you to sit for the NBHWC Certification exam preferred.
  • Established experience working with patients with chronic disease.
  • Demonstrated experience working within a medical model and/or an interdisciplinary team.
  • Demonstrated experience and comfortability working with patient data, including utilizing, and adapting to various technology platforms.
