Healthcare Engagement Coordinator

We are seeking a highly skilled Healthcare Engagement Coordinator to join our team at 3:15. In this role, you will play a vital part in improving patient care and fostering lasting relationships with healthcare providers. As a Healthcare Engagement Coordinator, you will serve as the primary point of contact for healthcare providers, ensuring effective communication and collaboration.
The ideal candidate will have at least 2 years of experience in a healthcare or patient engagement role, with excellent interpersonal and communication skills. You will also possess strong knowledge of healthcare regulations and best practices, as well as the ability to work independently and as part of a multidisciplinary team. If you are a motivated and detail-oriented individual with a passion for improving patient care, we encourage you to apply.
Responsibilities:
- Serve as the primary point of contact for healthcare providers
- Coordinate and implement patient engagement strategies to improve continuity of care
- Enroll eligible patients in the continuous care programs
- Assist with the coordination of medical equipment and supply delivery to patients' homes
- Document and maintain accurate and up-to-date patient records
