OBJECTIVE AND SCOPE:
Aligns with FMCNA's mission, vision, core values, and customer service principles. Complies with the FMCNA Compliance Program and adheres to all regulatory and policy requirements.


Acts as a vital member of the hemodialysis healthcare team, delivering safe and effective dialysis treatment for patients under the direct oversight of a licensed nurse, in accordance with FMCNA policies, procedures, and training, while adhering to state and federal regulations.

Responsible for the preparation and operation of hemodialysis machines. Contributes to maintaining a safe and hygienic working environment.

Participates in the FMCNA Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) initiatives, focusing on patient satisfaction and engaging in process improvement activities that support the achievement of FMC Quality Enhancement Goals (QEP).


RESPONSIBILITIES AND DUTIES:

CUSTOMER SERVICE:
Fosters the FMS culture through core values and customer service standards.

Ensures exceptional service to all internal and external customers.

Builds and sustains effective relationships through timely and clear communication.


Takes proactive measures to address, resolve, and follow up on customer service inquiries promptly.


KEY RESPONSIBILITIES:

PATIENT CARE:


Assists healthcare team members in delivering patient-specific education regarding adequacy measures when appropriate - including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), and Urea Kinetic Modeling (UKM).

Welcomes assigned patients and checks on their well-being since their last treatment. Reports any concerns or observations to the nurse supervisor.

Assesses vascular access for patency and communicates any unusual findings to the nurse supervisor.

Records necessary pre- and post-treatment vital signs and weight, reporting any irregularities to the nurse supervisor.

Conducts vascular access evaluations prior to treatment and informs the nurse supervisor of any unusual findings.

Executes vascular access cannulation and reports any challenges or unusual observations to the nurse supervisor.

Administers Heparin as delegated or permitted by state law.

Initiates dialysis treatment according to prescribed orders, including blood flow (QB) and dialysate flow (QD).

Identifies intradialytic issues and provides interventions as prescribed by physician orders or as directed.

Monitors patients' responses to dialysis therapy and communicates any unusual findings to the nurse supervisor.

Records vital signs prior to reinfusion; performs all necessary functions for treatment discontinuation - documenting appropriately.

Discontinues dialysis treatment following established protocols.

Evaluates patients prior to the termination of venous access - assessing standing and sitting blood pressure.

Achieves hemostasis and applies suitable dressings.

Assesses the patient prior to discharge and reports any unusual findings to the nurse supervisor.

Documents pre- and post-dialysis evaluation weight and vital signs with proper identification.


Responsible for calculating and entering individual patients' dialysis machine programming for Ultrafiltration (UF) goals, treatment duration, Sodium (Na) modeling, and UF modeling as prescribed.

Ensures that safety alarms are activated, including Narrow Venous Limits (NVL) 160.

Records accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with proper identification.


Responsible for precise documentation of patient treatment information, including completion of the Hemodialysis Treatment Sheet and, if applicable, progress notes.

Communicates any significant information and/or changes in patient condition directly to the nurse supervisor.

Observes patients and conducts machine safety checks in accordance with facility policy, reporting any changes or unusual findings to the nurse supervisor.

Documents any interventions for unusual patient status and records patients' responses to those interventions.


Evaluates patients' vascular access during treatment, including arterial and venous monitoring pressures, providing appropriate interventions as needed, and documenting and reporting any unusual findings to the nurse supervisor.

Collects all prescribed laboratory tests and prepares specimens for collection.

MAINTENANCE AND TECHNICAL:
Maintains environmental integrity and aesthetics - ensuring all areas are safe and clean.

Cleans and disinfects dialysis machine surfaces, chairs, equipment, and surrounding areas between treatments according to facility policy.

Completes all necessary tasks for dialysis treatment preparation and documents appropriately.

Conducts all required pretreatment dialysis machine alarm testing, including Pressure Holding (PHT).

Initiates the Solution Delivery System (SDS).

Ensures that all blood spills are promptly cleaned with the appropriate disinfectant as per facility policy.

Prepares, organizes, and efficiently utilizes supplies and equipment to minimize waste.

CLERICAL AND ADMINISTRATIVE:
Completes and documents ongoing participation in Continuous Quality Improvement (CQI) activities.

Accurately enters all treatment data into the designated clinical application in a timely manner.


Reviews treatment sheets for completeness, ensuring nursing signatures are documented and that any omitted entries are completed or corrected as necessary.

Prepares initial patient charts for admission and compiles paperwork for appropriate placement in the chart.

Prepares lab requisitions for prescribed or stat lab specimens according to laboratory destination.

Collects, labels, prepares, and stores lab samples according to required laboratory specifications.

Ensures the collection of lab specimens by the appropriate lab courier.

OTHER:
Performs additional duties as assigned.

Daily work includes desk and personal computer tasks and interaction with patients, facility staff, and physicians. The position requires travel between assigned facilities and various locations within the community. Travel to regional Business Unit and Corporate meetings may be necessary.

The work environment is typical of a healthcare facility with controlled air temperature and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur.

This position provides direct patient care that often involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This role requires frequent prolonged periods of standing, and the employee must be able to bend over. The employee may occasionally need to push and/or pull equipment, exerting up to 15 pounds of force.

The employee may be required to lift and lower solutions frequently, up to 30 lbs., and occasionally lift up to 40 lbs. as high as 5 feet. There is a two-person assist program and material assist devices for heavier items.



EDUCATION:
Center for Medicaid/Medicare Services (CMS)-approved state and/or national certification or High School diploma or G.E.D. and must meet certification requirements within the required state or CMS timeline.

All appropriate state licensure education and training (if any) required.

EXPERIENCE AND REQUIRED SKILLS:
Previous patient care experience in a hospital setting or related facility preferred.

Continued employment is contingent upon successful completion of the FMCNA dialysis training program and CPR certification.
