Care Manager

Atrium Health Charlotte is seeking a skilled Care Manager to coordinate with the medical team and facilitate optimal health outcomes for patients transitioning to post-acute care. The ideal candidate will possess strong interpersonal skills, critical thinking, and the ability to communicate effectively with patients, families, and healthcare professionals.
- Assess patient needs and develop individualized care plans to ensure seamless transitions.
- Collaborate with hospitals, clinics, and healthcare facilities to coordinate medical services and ensure continuity of care.
- Provide education and support to patients and their families to empower them to make informed decisions about their care.
- Work closely with clinical teams to identify and address potential barriers to care.
Requirements
- Graduate of an accredited school of nursing.
- Current RN license or temporary license as a Registered Nurse Petitioner in the state where the RN works.
- Minimum of three years nursing experience, preferably in acute healthcare or case management.
