Job Title: Licensed Practical Nurse Charge Nurse PRN

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Charge Nurse PRN at First Community Village Senior Living Campus in Columbus, OH.

Job Summary:

The LPN Charge Nurse will be responsible for providing high-quality nursing care to our residents, supervising nursing assistants, and ensuring the efficient completion of their assignments. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Supervise and direct nursing care in compliance with facility policies and procedures.
  • Make observations, report, and record observations appropriately, form nursing judgments, and carry out nursing actions based on understanding of scientific principles involved.
  • Follow established standards of nursing practices, assume responsibility for compliance with federal, state, and local regulations, and interpret existing policies to staff, families, and physicians.
  • Perform daily rounds, develop and implement accurate comprehensive care plans based on each resident's needs, and follow established policies and procedures for passing medications and treatments.
  • Follow staffing guidelines, make nursing assignments, receive and give an accurate report on resident's condition to provide continuity of care, and monitor diets and supervise meal trays and snacks served to residents and their intake.
  • Assist in passing meal trays, feeding, and passing of snacks, follow facility admissions procedures, and communicate admission to other departments, staff, physicians, and other service providers.
  • Ensure employees follow the chain of command in a manner consistent with the personnel policies, assign nursing assistants, and ensure the efficient completion of their assignments.
  • Supervise, document, and correct poor performance of nursing assistants, keep Unit Managers/Director of Nursing informed on a timely basis of poor performance problems, and participate in the performance evaluation process of nursing assistants and provide counseling, coaching, and training.
  • Present performance evaluations to staff, communicate professionally and effectively, follow written and verbal directions, provide exemplary customer service to residents, peers, families, managers, and visitors, and report any changes in physical or mental conditions on a timely basis.
  • Follow all company, facility, and state policies and procedures, attend staff meetings, in-services, inter-disciplinary team, and committee meetings as assigned, exercise keen observations of noise, odors, cleanliness, etc., cooperate with other staff members in all departments to ensure quality environmental conditions for the residents and staff, and document resident information timely, accurately, and confidentially according to procedures.
  • Document changes and notification to POA and doctor, perform comprehensive assessments as assigned, verify change-over MARS/TARS are correctly updated and necessary changes are made to new MAR/TAR by the 1st of each month, adhere to the guidelines as outlined in the Resident Rights documentation, know resident rights, and help the residents exercise and/or protect their rights.
  • Report resident complaints to supervisor in charge, maintain confidentiality, adhere to all policies and procedures as outlined under the Safety & Compliance policies and procedures, and ensure that residents are safe, for example, Wanderguard tab is securely attached to bed/chair as ordered, Wanderguard bracelets are working properly.
  • Demonstrate proper use of equipment, report equipment needs or repairs, use protective equipment, follow infection control protocol standards, policies, and procedures, practice universal precautions and life safety measures.
