Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Centralia Hospital. As a Clinical Pharmacist, you will play a critical role in providing high-quality, individualized pharmaceutical care to our patients.
Key Responsibilities- Develop and implement medication therapy plans to optimize patient outcomes
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication
- Conduct thorough medication reviews and provide recommendations for improvement
- Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care
- PharmD or Bachelor's Degree in Pharmacy
- Washington Pharmacist License
- Basic Life Support-Provider (BLS) certification
- Acute care hospital pharmacy experience preferred
