About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Centralia Hospital. As a Clinical Pharmacist, you will play a vital role in providing high-quality, individualized pharmaceutical care to our patients.

Responsibilities
  • Provide clinical pharmacy services, including medication therapy management and patient education.
  • Collaborate with the healthcare team to optimize patient outcomes and quality of life.
  • Develop and implement individualized medication plans to meet the unique needs of each patient.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
Requirements
  • PharmD or Bachelor's Degree in Pharmacy.
  • Current Washington State Pharmacist License.
  • Basic Life Support-Provider (BLS) certification.
  • Preferred qualifications include acute care hospital pharmacy experience and completion of an ASHP-accredited PGY1 residency.
