About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Centralia Hospital. As a Clinical Pharmacist, you will play a critical role in providing high-quality, individualized pharmaceutical care to our patients.

Key Responsibilities
  • Develop and implement medication therapy plans to optimize patient outcomes
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Conduct thorough medication reviews and provide recommendations for improvement
  • Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care
Requirements
  • PharmD or Bachelor's Degree in Pharmacy
  • Washington Pharmacist License (upon hire)
  • Basic Life Support-Provider (BLS) certification (where applicable)
Preferred Qualifications
  • Acute care hospital pharmacy experience (1 year or equivalent)
  • Completion of ASHP-accredited PGY1 residency
