Job Title: Community Support Specialist

At Seven Counties Services, we are committed to providing high-quality care and support to individuals with mental health and co-occurring disorders. We are seeking a compassionate and dedicated Community Support Specialist to join our team.

Job Summary:

The Community Support Specialist will provide therapeutic support and guidance to individuals in a positive treatment environment, promoting healthy behaviors and encouraging independence, socialization, and normalization within the community.

Key Responsibilities:
  • Teach and model activities of daily living (ADL) skills, such as personal hygiene, housekeeping, and meal preparation.
  • Plan and facilitate community-based activities, including medical appointments, grocery shopping, and leisure/recreation/socialization.
  • Contribute to a safe and therapeutic environment by practicing verbal de-escalation and other behavior management techniques.
  • Collaborate with community systems on behalf of individuals served, including schools, landlords, and transportation providers.
  • Provide transportation, ensuring adherence to standard safety practices, and teach and facilitate use of public transportation systems.
  • Administer First Aid and CPR, as needed, and supervise self-administered medication.
  • Communicate information relevant to care, treatment, and services with other SCS staff members and non-SCS providers.
  • Document services and care provided, maintaining confidentiality of protected health information.
Requirements:
  • Up to 18 months' training, beyond high school or GED, in direct care of individuals with disabilities.
  • Completion of all required trainings within 6 months of hire date.
  • One year full-time experience working with individuals who receive services for treatment of mental health or co-occurring disorders.
  • Formal documentation by supervisor of ability to function independently as a direct support professional and proficiency in providing direct support services.
