As a LPN, you will play a vital role in helping patients progress toward independent living.
Key Responsibilities- Administer medications accurately and safely, considering the age and clinical condition of the patient.
- Provide patient care that meets the standards of care for geriatric and general inpatient rehabilitation patients.
- Assist with patient assessments and reassessments, identifying and initiating appropriate rehabilitation nursing interventions under the supervision of the Registered Nurse (R.N.).
- Perform timely and accurate Quality Improvement (QI) assessments and update the interdisciplinary care plan accordingly.
- Develop and implement individualized plans of care that address the unique needs and goals of each patient.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Manage pain effectively and educate patients and families on pain management strategies.
- Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Current state licensure as a Licensed Practical/Vocational Nurse.
- CPR/BCLS certification; ACLS certification preferred.
- IV Certified or obtain certification within 6 months, highly preferred.
- Minimum six months of Medical/Surgical experience in an acute care setting, preferred.
- Excellent oral and written communication and interpersonal skills.
