Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and develop personalized care plans to improve health outcomes for our members.

Key Responsibilities
  • Conduct in-home assessments, medical history reviews, and develop personalized care plans to improve health outcomes for members
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to members and their caregivers to promote healthy behaviors and disease management
  • Work with our care management team to identify and address social determinants of health that impact member outcomes
  • Document all assessments, care plans, and interventions in our electronic health record system
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record systems
  • Excellent verbal and written communication skills
Travel Requirements

This role requires frequent travel to member homes, with occasional travel to our office for meetings and training. A valid state driver's license and adequate automobile insurance are required.
