Pediatric Healthcare Specialist

We are seeking a highly skilled and compassionate Registered Nurse (RN) to provide quality healthcare to patients in their homes. As a Home Health Nurse, you will be responsible for delivering patient-centered care with a focus on pediatrics.
The ideal candidate will have minimum 1 year of experience in home care with pediatrics, as well as expertise in peg tube feeding/care, medication administration, and seizure precautions. You will work collaboratively with our multidisciplinary team to develop and implement comprehensive care plans tailored to each patient's unique needs.
Responsibilities:- Participate in the development and revisions of patient-specific assessments and care plans.
- Provide patient and family education related to disease processes and care needs.
- Collaborate with care teams, including physicians, supervisors, coworkers, and other medical providers.
- Record and report changes in patients' conditions.
- Communicate updates of medical condition to Physician and Clinical Director.
