Job Summary

We are seeking a skilled Patient Care Partner to join our nursing team. As a key member of our healthcare team, you will work closely with patients, families, and healthcare professionals to deliver high-quality care and support.

Responsibilities
  • Provide direct patient care, including lifting, turning, and positioning patients, as well as assisting with daily living activities.
  • Observe and report changes in patient condition to nursing staff.
  • Participate in patient safety and satisfaction initiatives.
  • Assist with patient transfers and ambulation, using Safe Patient Handling equipment as needed.
  • Perform and record vital signs, including temperature, pulse, respirations, and blood pressure.
  • Collect and process patient specimens, including urine, stool, and sputum.
  • Transport patients and equipment as needed.
  • Assist with patient education and discharge planning.
  • Participate in performance improvement activities to enhance patient care and service.
Requirements
  • High school graduate or equivalent.
  • Successful completion of a CNA training course or minimum of 1 year experience in a nurse aide or equivalent role.
  • Current enrollment in a licensed nurse education program and successful completion of a fundamentals of nursing course.
