About the Role

We are seeking a highly skilled Discharge Nurse to join our team at Orlando Health. In this role, you will be responsible for preparing patients for discharge from our hospital by ensuring they have a clear understanding of their post-discharge care and instructions.

Responsibilities
  • Collaborate with patients' nurses to review their charts and determine the necessary preparations for discharge.
  • Determine where patients are going after discharge, whether directly home or to another care facility, and ensure they have transportation arrangements in place.
  • Evaluate patients' needs for medical equipment at home, such as glucose monitoring items, wheelchairs, or medications, and arrange for delivery or pickup as needed.
  • Complete all necessary discharge paperwork, including medication teaching and education with patients and their families.
  • Transport qualified patients to the discharge lounge.
Requirements
  • A graduate of an approved school of nursing with a current State of Florida RN license or valid eNLC multistate RN license.
  • At least one year of experience as a Registered Nurse in a hospital setting with Epic discharge experience.
  • Current BLS/Healthcare Provider certification.
