This role is an exciting opportunity for a Medical Care Liaison Specialist to join our team. As the Organization's representative, you will facilitate referrals in facilities while working closely with referring hospitals, physicians, facilities, patients, families, and the general public.

We offer a competitive salary of $75,000 per year, which is commensurate with your experience and skills. This position requires a current Florida license as RN, or a current active single-state out-of-state nurse license, or a current active multistate/compact nurse license.

The ideal candidate will have at least one year of acute healthcare experience, preferably in hospice care. You should also have previous experience working with an EMR/EHR system and be able to effectively communicate in English (written and verbal).

In this role, you will be responsible for presenting services in an empathetic and compassionate manner, obtaining consents for services, and arranging safe discharge for referred clients. You will work closely with case managers, physicians, and other healthcare professionals to develop strong relationships and provide education on available services.

This position requires ability to multitask, prioritize, and manage physical demands and emotional stress of workload. You should also have strong goal/outcome orientation with exceptional time-management skills. A valid driver's license and automobile insurance are required, as well as reliable transportation to meet schedule.
