Job Overview

We are seeking a highly skilled Mental Health Nurse Practitioner to join our team at Supportive Care, a leading provider of behavioral health services to skilled nursing facilities, assisted living facilities, and memory care communities throughout the United States.

Key Responsibilities:

  • Diagnose and treat acute, episodic, or chronic mental health conditions in patients
  • Prescribe and manage medications and treatments to ensure optimal patient outcomes
  • Conduct comprehensive physical and mental health assessments to develop personalized patient care plans
  • Collaborate with healthcare professionals to provide seamless patient care and support
  • Develop and implement policies and procedures to ensure high-quality patient care and services
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes

Requirements:

  • Previous experience in mental health nursing or a related field
  • Strong leadership and communication skills to build rapport with patients and healthcare teams
  • Ability to work in a fast-paced environment and adapt to changing patient needs
  • Excellent written and verbal communication skills to effectively communicate with patients, families, and healthcare teams
  • Strong analytical and problem-solving skills to develop and implement effective patient care plans
