Position Overview:
The Nurse Coordinator is responsible for providing direction, coordination, supervision, and implementation of professional and supportive services to Home Care Agency (HCA) consumers. This role involves monitoring services delivered to HCA consumers, training attendants, and ensuring consumer satisfaction and safety. Additionally, the Nurse Coordinator will complete and submit all necessary documentation in compliance with state and federal regulations.
Key Responsibilities:
1. Communicate effectively with team members and clients.
2. Maintain accurate medical documentation.
3. Conduct annual in-person supervisory visits.
4. Perform record audits.
5. Provide education to clients, employees, and families.
6. Utilize the Carevoyant software program.
Qualifications:
- Minimum of 1 year of experience as a Registered Nurse (RN) is required, with a preference for additional experience in acute care or medical/surgical nursing.
- Home health nursing experience is preferred.
- At least 1 year of supervisory experience overseeing nurses and other care providers.
- Spanish language proficiency is preferred.
Education and Licenses:
- Current, unencumbered RN license in the state of Colorado.
- Bachelor of Science in Nursing (BSN) is preferred.
- Current CPR and First Aid certification.
Compensation:
Pay range is $33 - $36.50 per hour, plus mileage reimbursement and a $2,000 sign-on bonus.
Work Environment:
This position offers a hybrid working arrangement, requiring two days in the PASCO office.
