With a focus on quality of care, compliance, and outcomes, you'll work to develop and implement strategies to coordinate and integrate post-acute and long-term care services. You'll also conduct regular assessments and updates with providers, primary care physicians, and other stakeholders to ensure our patients receive the best possible care.

Key Responsibilities:

  • Participate in care planning and coordination with the care management team
  • Conduct regular assessments and updates with providers and stakeholders
  • Develop and implement strategies to coordinate and integrate post-acute and long-term care services
  • Collaborate with hospital social workers and community resources to ensure seamless care transitions
  • Identify and report potential quality improvement issues

Requirements:

  • Active and unrestricted LSW or LCSW licensure in the state of Pennsylvania
  • Minimum of 2+ years experience in medical social work or case management
  • Ability to travel 50-75% within the designated geographic area
