Job Title: Behavioral Health Case Manager

Adelante Healthcare is seeking a skilled Behavioral Health Case Manager to join our team. As a Behavioral Health Case Manager, you will be responsible for managing an assigned caseload of clients with long-term mental illness and assessing client needs, developing, implementing, and reviewing service plans.

Key Responsibilities:
  • Participate in the screening of new service requests and ensure that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures.
  • Develop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients.
  • Provide direct client services to individuals and/or family members, including case management, skills training, group counseling, group skills training, health promotion, crisis intervention, transportation, supportive employment services, counseling with appropriate clinical oversight, and others depending on client need.
  • Provide on-going supportive and/or case management functions in accordance with the problems, needs, and strategies identified within the service plan to assist the client to achieve goals and objectives.
  • Document all service contacts on a timely basis, including face-to-face interviews, collateral and networking contacts, correspondence, and maintain the case records in accordance with internal and regulatory standards requirements.
Requirements:
  • Bachelor's Degree in Social Service field or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelor's Degree.
  • High school diploma or GED from an accredited institution is required.
  • Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferred.
  • Excellent customer service skills.
  • Excellent computer skills.
  • Knowledge of Arizona Department of Health Services and RBHA requirements.
  • Thorough knowledge of current versions of DSM and ICD.
  • Ability to complete a Behavioral Health Assessment with appropriate clinical oversight.
  • Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices.
  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross.
  • Bilingual English/Spanish strongly preferred.
  • Valid Arizona driver's license.
  • Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety.
Working Environment:

The Behavioral Health Case Manager will work in a fast-paced environment with a team of healthcare professionals. The ideal candidate will be able to work independently and as part of a team, with excellent communication and interpersonal skills. The Behavioral Health Case Manager will be required to travel to various clinic sites and may be exposed to health hazards (contagious diseases, blood-borne pathogens, etc.) when working in the clinic area.
