Job Title: Case Manager - Children's Behavioral Health

We are seeking a compassionate and experienced Case Manager to provide case-management services to children receiving behavioral-health services across The Valley. As a Case Manager, you will work closely with families and caregivers to provide guidance and support as needed.

Responsibilities:
  • Provide weekly contact with assigned clients, including home/community visits and attending medication checks.
  • Assist youth in navigating discharge from treatment settings and finding services as clinically indicated.
  • Coordinate services and collaborate with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Coordinate and facilitate Child & Family Team (CFT) Meetings.
  • Collaborate with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
Qualifications:
  • Meet one of the following education/experience requirements:
  • High School Diploma/GED required and 4 years of Behavioral Health experience.
  • A.A. Degree and two years of Behavioral Health experience.
  • Bachelor's Degree in Social Work, Psychology, or related field.
  • Additional qualifications:
  • Level 1 Fingerprint Clearance Card without Restriction(s).
  • Meet Agency requirements for MVR (Motor Vehicle Record) report.
  • Reliable transportation.
  • AZ driver's license.
  • AZ auto insurance.
