Consequently, the company is striving to extend our distinguished, quality outcomes to as many patients as possible through our outpatient clinic partnerships.

Role Overview: The Patient Navigator is responsible for a variety of front office functions and plays a crucial role in the seamless operation of a dynamic outpatient wound care center. This position also entails performing general office duties to assist the staff of the Wound Care Center (WCC).

Key Responsibilities:

  • Collaborates with Center leadership to optimize daily patient attendance.
  • Actively engages in staff meetings to support essential functions within the Center.
  • Welcomes patients and visitors, answering and directing calls to the appropriate personnel.
  • Manages all patient communication needs, including scheduling, rescheduling, and appointment reminders, while coordinating with providers according to the care continuum model.
  • Obtains and verifies patient insurance details, including pre-certifications and pre-authorizations for services, and enters data into relevant databases.
  • Collects and records patient charges in databases.
  • Verifies and reconciles charges as instructed, prepares monthly patient survey data and necessary documentation, then transmits to providers.
  • Assembles new patient charts, maintains and files existing patient charts, and conducts spot checks for data completeness and signatures.
  • Coordinates and schedules ancillary testing with other hospital departments.
  • Arranges patient transportation as necessary.
  • Maintains office equipment and supplies, as well as medical supplies as directed.
  • Performs additional duties as required.
Required Qualifications:
  • High School Diploma or General Education Development (GED); Associate's degree preferred.
  • A minimum of two (2) years of office administration experience, preferably in a medical environment; prior medical coding experience is advantageous.

  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Strong customer service, interpersonal, and communication skills, both verbal and written.
  • Excellent organizational and time-management abilities.
  • Typing speed of 60 words per minute (wpm).
  • Basic mathematical skills.
  • Attention to detail.
  • Ability to maintain confidentiality.
  • Capability to thrive in a fast-paced environment and manage multiple projects simultaneously.
  • Ability to collaborate effectively within a team.
Physical Requirements:
  • Ability to remain in a stationary position for extended periods (4 hours or more).
  • Prolonged viewing of computer screens (4 hours or more).
  • Frequent keying on a computer for 4 hours or more.
  • Reading and communicating effectively.
  • Detecting sounds by ear.
  • Close, distance, and peripheral vision.
  • Lifting/moving items up to 75 pounds with equipment assistance.
  • Repetitive motions and bending/stooping.
  • Writing.
Work Environment:
  • Patient care setting.
The hourly rate for this position typically ranges between $18.84-$24.55 per hour. This range is an estimate based on potential employee qualifications, including education, experience, geography, and operational needs, as well as other considerations permitted by law.
