Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care team at Commonheart. As a RN, you will provide high-quality patient care and support to individuals in their homes, focusing on comfort, dignity, and well-being.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Develop and implement education plans to promote patient understanding and self-management of their conditions.
  • Document patient progress and communicate effectively with the healthcare team.
Requirements
  • Current RN licensure in the State of Wisconsin.
  • Experience in hospice or home health care preferred.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to navigate electronic medical records and other healthcare software.

Work Environment
