Registered Dietician

St Croix Hospice is seeking a skilled Registered Dietician to join our team. As a Registered Dietician, you will play a vital role in providing nutritional care to our patients and their families.

The Registered Dietician will plan, organize, and conduct dietetic education for nursing personnel and other personnel. You will function as a member of the interdisciplinary team with expertise in nutritional care.

• Develop and implement a nutritional plan of care based on a comprehensive assessment of nutritional needs.
• Evaluate and report results appropriately.
• Work with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers in the home setting.
• Evaluate and instruct on nutritional principles, dietary plans, food selection and economics, and adapting plans to the individual's illness, as indicated.
• Reevaluate nutritional care by providing home follow-up care either at the patient's residence or by phone.
• Communicate appropriate dietary history and nutritional care data through written record systems.
• Record subjective and objective information, assessment, treatment plan, and current progress notes in the patient's clinical records including a discharge summary.
• Attend interdisciplinary group meetings.
• Comply with the organization's policies and procedures.
• Maintain confidentiality and safety of patient records.
• Actively participate in quality assessment performance improvement teams and activities.

We are seeking a Registered Dietician with a Master's degree from an accredited institution with a major in foods, nutrition, or dietetics, and successful completion of an internship or equivalent experience. You must also be a member and registered in the American Dietetic Association, and licensed by appropriate state(s), if applicable. Additionally, you must possess good verbal and written communication, and organization skills, and possess and maintain current CPR Certification.
