Nurse Care Coordinator

Job Summary:
CareOne is seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality patient care and leading our interdisciplinary team in delivering exceptional outcomes.
Key Responsibilities:
- Provide direct nursing care to assigned residents in accordance with the nursing process.
- Assess and respond to resident conditions utilizing the nursing process.
- Work cooperatively with other members of the facility interdisciplinary team.
- Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures.
- Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property.
Requirements:
- Associate's Degree from credentialed school of nursing.
- Currently licensed as a Registered Nurse in the state of practice.
- 1 year of related nursing experience.
