Job Summary

St. Elizabeth Physicians is seeking a highly skilled RN Care Coordinator to join our team. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our patients with complex needs.

Key Responsibilities
  • Collaborate with healthcare providers, interdisciplinary staff, and clinical associates to develop and implement comprehensive care plans.
  • Facilitate effective communication and coordination of services to address patient needs and barriers.
  • Provide education and guidance to patients and their families on current health concerns and self-management strategies.
  • Work closely with care teams to ensure timely and efficient transitions of care.
  • Participate in quality improvement initiatives to enhance patient outcomes and care coordination processes.
Requirements
  • Current State Allied Health License required.
  • Three to four years of medical field experience, preferably in ambulatory or care management settings.
  • Demonstrated knowledge of anatomy, physiology, pharmacology, and electronic health records (EPIC).
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Positive attitude, warm personality, and a commitment to delivering exceptional patient care.
Preferred Qualifications
  • Care Management Certification.
  • Experience with motivational interviewing and patient education.
  • Knowledge of Excel, Word, Outlook, and PowerPoint.
