Pulmonary Hypertension Registered Nurse Coordinator Job Description

University Physicians Association is seeking a highly skilled and detail-oriented Pulmonary Hypertension Registered Nurse Coordinator to join our team at University Pulmonary & Critical Care located within the UT Medical Center. As a Pulmonary Hypertension Registered Nurse Coordinator, you will play a vital role in the assessment, treatment, and coordination of care for patients with Pulmonary Hypertension and other pulmonary conditions in the office/clinical setting.

Key Responsibilities:
  • Coordinate outpatient PH Clinic processes, ensuring seamless patient flow and efficient use of resources.
  • Develop and implement PH protocols, staying up-to-date with the latest clinical guidelines and best practices.
  • Coordinate prescription management, ensuring accurate and timely medication administration.
  • Coordinate patient transition from inpatient to outpatient, ensuring a smooth and safe transition.
  • Coordinate nursing/staff PH education, providing training and support to ensure high-quality patient care.
  • Represent the PH Clinic at national and local meetings, staying informed about the latest developments in PH care.
Requirements:
  • Current Registered Nurse (RN) license.
  • At least six months of experience in a similar clinical setting.
  • Minimum associate-level RN degree.
  • Knowledge and ability to perform basic clinical skills, including phlebotomy, injections, IV therapy, and various testing as needed.
  • Ability to adjust work pace to facilitate clinic flow, ensuring efficient use of time and resources.
  • Excellent verbal communication skills, with the ability to effectively communicate with patients, staff, and physicians.
  • Ability to apply policies and procedures to daily scenarios, dealing with a variety of challenges and situations.
