The Opportunity

">

We're seeking a full-time Home Visit Nurse Practitioner to join our high-risk home-based care team in Metro/West Atlanta, GA. As a key member of our team, you'll care for geriatric, frail, and clinically complex patients in partnership with our High Risk Care Medical Director, Health Coaches, Care Management Nurses, Social Workers, Integrated Behavioral Health Specialists, and practice-based PCPs to provide compassionate, human-centric care.

  • Delivering high-value care to vulnerable patients">
  • Rediscovering the joy in practice in a value-based care environment">
  • Independently managing a panel of approximately 150 case-mix adjusted patients with multiple, complex conditions through a balance of home visits and telephone/video interventions">

Key Responsibilities:

">
  • ">
  • Completing thorough initial visits with patients, diving deep into medical, social, and behavioral context">
  • Working directly with patients and through our multidisciplinary care team to impact ongoing care, particularly emphasizing preventable hospitalizations and ensuring care aligned with patient goals">
  • Collaborating and coordinating with patients' primary care teams and specialists">
  • Participating actively in team huddles to review care plans and assess patient goals of care and care plans">
  • Responding to patient and patient family telephone calls">
  • Participating in our market's after-hours call pool">

Requirements:

">
  • ">
  • 2+ years of home-based complex care, in-office complex care, Geriatric Primary Care, Geriatrics, or Palliative Care experience as a Registered Nurse or Nurse Practitioner AND 6+ months of NP experience">
  • Current licensure in the state of Georgia">
  • Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult">
  • Experience with home-based care preferred">
  • Unrestricted driver's license and ability to drive to patient homes">
  • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups">
  • Able to thrive in a fast-growing, mission-driven organization focused on using data to improve patient outcomes">

Estimated Salary: $115,000 - $140,000 per year, depending on qualifications and experience
