Geriatric Home Health Nurse Practitioner

We are seeking a highly skilled and compassionate Geriatric Home Health Nurse Practitioner to join our One Medical at Home team in Metro/West Atlanta, GA.
This role is designed to provide high-quality care to geriatric, frail, and clinically complex patients in the comfort of their own homes.
Responsibilities- Care for patients with complex medical needs through a collaborative partnership with the High Risk Care Medical Director, Health Coaches, Care Management Nurses, Social Workers, Integrated Behavioral Health Specialists, and practice-based PCPs.
- Work closely with the multidisciplinary care team to develop and implement comprehensive care plans tailored to each patient's unique needs.
- Conduct thorough assessments and evaluations to identify patients' healthcare needs, preferences, and goals.
- Prioritize timely interventions to prevent hospitalizations, reduce readmissions, and improve overall patient outcomes.
- Maintain accurate and detailed records of patient interactions, treatment plans, and progress notes.
To be considered for this rewarding opportunity, candidates must possess:
- A Master's degree in Nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited institution.
- A valid Advanced Practice Registered Nurse (APRN) license in the state of Georgia.
- Current certifications as a Family Nurse Practitioner (FNP) or Gerontological Nurse Practitioner (GNP).
- Proven experience working with geriatric patients and managing complex medical conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.

The estimated annual salary range for this position is $120,000 - $150,000, depending on experience and qualifications.
