Job Description

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Longleaf Hospice. As a key member of our medical team, you will work collaboratively with physicians to deliver high-quality patient care in various settings.

  • Develop and implement palliative care treatment plans for patients in all settings, including history, physical examination, interpretation of health data, and prescribing medications as needed.
  • Communicate effectively with medical colleagues, patients, and families to provide comprehensive care and support.
  • Provide education and counseling on healthy behaviors, wellness visits, and referrals to other healthcare professionals and community resources.
  • Assess patient progress, adjust treatment plans as necessary, and maintain accurate records to ensure sound medical practice and reimbursement.
  • Participate in interdisciplinary team meetings, provide training and continuing education for staff, and contribute to the development of clinical policies and procedures.

Requirements:

  • Current unrestricted license to practice as a registered professional nurse in the state of employment.
  • Graduate of an approved Nurse Practitioner program.
  • Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
  • Current and continued CPR certification.
  • Certified Hospice and Palliative Care Nurse (CHPN) preferred.
  • Minimum of 3 years of palliative care and/or hospice experience required.

Salary: $120,000 - $150,000 per year, depending on experience.
