Licensed Vocational Nurse

As a Licensed Vocational Nurse with Sutter Health, you will provide high-quality nursing care to patients in their homes, promoting independence and well-being. Responsibilities:
• Provide nursing care to patients in their homes, following established protocols and procedures.
• Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans.
• Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
• Educate patients and their families on disease management, medication use, and other health-related topics.
• Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services.
• Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
Requirements:
• Graduate of an accredited school of Vocational Nursing.
• Current LVN licensure in California.
• Basic Life Support (BLS) certification.
• Blood draw certificate within 30 days of hire.
• Valid California Driver's License and automobile insurance.
Preferred Qualifications:
• 1 year of experience in a similar environment, preferably in home health care.
Skills and Knowledge:
• Demonstrated knowledge of basic nursing skills, medical procedures, and equipment.
• Ability to interpret data and instructions, including written, oral, and diagrammatic information.
• Basic understanding of human anatomy, physiology, pathology, and medical terminology.
• Excellent written and verbal communication skills, with the ability to explain complex information to diverse audiences.
• Strong time management and organizational skills, with the ability to prioritize tasks and work independently.
• Familiarity with computer applications, such as Microsoft Office Suite.
Work Environment:
• This position may require regular work in a home health setting, with occasional travel to Sutter Health facilities.
• The nurse will be expected to work independently, with occasional collaboration with other healthcare professionals.
• The nurse will be required to maintain confidentiality and adhere to all relevant laws, regulations, and Sutter Health policies.
