Job Description:

This is a part-time swing shift position at Sherwood Pines Memory Care, a specialized adult foster home that provides individualized care in a home-like environment. Our mission is to provide the best care possible for our residents with adult-onset disease processes, such as neuromuscular disease, TBI, complex medical and psychiatric issues.

Responsibilities:
  • Must be able to pass a background check and complete dementia and caregiver preservice training.
  • Assist residents with activities of daily living, including toileting, bathing, dressing, hygiene, and transferring.
  • Assist residents with hydration, meal setup, and eating assistance.
  • Encourage residents to engage in person-centered activities and participate with other residents as appropriate.
  • Treat all residents with respect, dignity, and compassion.
  • Support and perform care as specified in the resident service plan.
  • Observe residents for changes in condition and report any significant changes.
  • Respond to emergency situations according to current policies and procedures.
  • Complete incident reports per policy on assigned shift.
  • Take and record vital signs into resident charts.
  • Promptly respond to calls.
  • Maintain a clean and tidy environment, including resident rooms and common areas.
  • Change linens and remove garbage.
  • Complete laundry and ensure it is delivered back to the resident room in a timely manner.
  • Transfer and escort residents following care plans and safe practices.
  • Document care provided.
  • Contribute to shifting reports and care notes to ensure continuity of care.
  • Administer medications after medication class and approval from the Administrator.
  • Report to the Administrator any signs of resident abuse or witnessed misappropriation.
  • Adhere to the dress code and maintain a neat and clean appearance.
  • Perform other duties as assigned or delegated by the nurse verbally or in writing.
