Job Summary:

We are seeking a skilled Social Work Specialist to join our team as a Patient Care Advocate. In this role, you will play a critical part in assessing the psychosocial needs of patients and their families, providing them with vital information, support, counseling, care management, and referrals to relevant resources.

Job Description:

As a Social Work Specialist, you will be part of an interdisciplinary team of physicians, nurses, and other professionals working together to ensure continuity of patient care. Your responsibilities will include:

  • Conducting Comprehensive Psychosocial Assessments: You will assess the psychosocial needs of patients and their families, identifying areas where support and counseling are required.
  • Developing and Implementing Care Plans: Based on your assessments, you will work with patients and their families to develop care plans, ensuring that their needs are met and they receive the support they require.
  • Providing Counseling and Support: You will provide counseling and support to patients and their families, addressing social, psychological, financial, and other issues that may impact their care.
  • Facilitating Resource Planning: You will assist patients and their families in accessing resources, including healthcare services, financial assistance, and other support.
  • Acting as a Patient Advocate: You will act as a patient advocate, communicating their needs and concerns to healthcare providers and advocating on their behalf.
Requirements:

To be successful in this role, you will need:

  • Master's degree in Social Work (MSW) from an accredited program
  • Two years of social work experience, preferably in a hospital setting

Salary:

The estimated salary for this role is $ $63,420 per year, based on national averages and industry standards.
