Key Responsibilities
  • Ensure the use of an interdisciplinary approach to assess the medical, physical, social, emotional, and spiritual needs of patients and their families.
  • Participate in team meetings, group management meetings, family meetings, and patient calls to ensure seamless care delivery.
  • Monitor and audit files, processes, and procedures to ensure regulatory compliance with Hospice Conditions of Participation and standards.
  • Collaborate with Quality Improvement and Education to ensure professional practice standards are consistently met for each team member.
  • Initiate, coordinate, and perform analysis of clinical, regulatory, and fiscal practice to support the achievement of Hospice Care goals and objectives.
Requirements
  • License and current registration to practice as a Registered Professional Nurse in New York State.
  • Bachelor's degree in nursing from an approved program accredited by the National League of Nursing or Health or Human Services field.
  • Minimum three years of clinical nursing experience, preferably in a home care or hospice setting.
  • Clinical management experience is preferred.
