Aveanna Healthcare Private Duty Nurse

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse - LVN to join our team. As a Private Duty Nurse, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes.
Key Responsibilities- Implementation of Physicians' Prescribed Plan of Care: Develop and implement personalized care plans in collaboration with our patients' physicians.
- Patient Assessments and Coordination of Care: Conduct thorough assessments of our patients' needs and coordinate care with our multidisciplinary team.
- Administration of Prescribed Medication, Treatments, and Therapies: Safely administer medications, treatments, and therapies as prescribed by our patients' physicians.
- Compliant, Timely Documentation of Care Provided: Accurately document all care provided in our electronic system of record.
- Health Promotion, Teaching, and Training of Family Members: Educate our patients' family members on health promotion, disease prevention, and management techniques.
- Responsible for Accepting and Writing Verbal Orders from Physicians: Accept and write verbal orders from physicians as required, and obtain co-signature of Case Manager on all verbal orders.
- Follow-up with Physician, Client, Family Members, Staff, and Case Manager: Communicate effectively with our patients' physicians, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition.
- Active, Unencumbered License (RN/LPN/LVN): Must have and maintain an active, unencumbered license in the state in which the clinician will practice.
- Current CPR Certification: Current CPR certification from American Red Cross, American Heart Association, or BLS.
- TB Skin Test (Current within Last 12 Months): Must have a current TB skin test within the last 12 months.
- Six Months Prior Hands-on Nursing Experience Preferred: Six months prior hands-on nursing experience preferred but not required.
- Reliable Transportation: Must have reliable transportation.
