We are seeking a skilled Licensed Practical Nurse to join our team at Lutheran Homes Society, Inc. in an Assisted Living setting. The successful candidate will provide high-quality care to residents, work collaboratively with the care team, and contribute to a positive and supportive work environment.
Key Responsibilities- Provide primary direction to the Resident Aides on assigned unit/floor/wing to ensure effective care is provided to residents.
- Prepare and administer medications and treatments to residents.
- Perform assessments, including vitals, intakes, outputs, etc.
- Perform direct care functions, assisting with ADLs and ensuring residents' spiritual, emotional, and physical needs are met.
- Process physician's orders, labs, x-rays, etc., and seek clarification when necessary.
- Communicate effectively with physicians, pharmacists, family members, etc., to discuss ongoing care of residents.
- Process new resident admissions and communicate admission information to department heads, as appropriate.
- Complete paperwork, medical forms, charting, and all required documentation as part of the resident care process and transition of care process.
- Complete change of shift duties.
- Assist in maintaining infection control programs, policies, and procedures.
- Report allegations of abuse/neglect and report, as appropriate, to proper organization leadership.
- Report hazardous incidents, including but not limited to, injuries, equipment, etc., to the appropriate Manager/Supervisor.
- Implement and follow disaster plans in the event of an emergency.
- Participate in committees as assigned.
- Perform other job duties as assigned.
- Current State of Ohio LPN License.
- 1-2 years of experience in long-term care setting preferred.
- Familiarity with basic computer skills.
- Ability to learn new software applications as required by the position.
- Excellent oral and written communication skills, including the ability to interact with senior executives, managers, supervisors, and direct care staff, as well as residents, families, and physicians.
- Understanding of directions, communication, and response to inquiries.
