**Key Responsibilities:**

* Provide support to referral families through home-based services and resource connections
* Collaborate with community agencies and resources to advocate for at-risk children and families
* Maintain relationships with social workers and agencies, including school systems and other institutions
* Facilitate goal-setting processes for referral families
* Connect families with local community resources

**Requirements:**

* Bachelor's degree in social work, psychology, sociology, or a related human service field
* 2+ years of experience in social service for children or families
* Experience in child development
* Knowledge of community resources programs
* Ability to recognize and respond to situations of abuse or neglect
* Valid driver's license with valid insurance and no more than 6 points on driving record
