About the Job

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a member of our multidisciplinary healthcare team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans that promote our clients' health, safety, and independence.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function
  • Collaborate with nurses and other healthcare professionals to revise and update care plans as needed
  • Educate clients and their families on rehabilitative care and activities to promote independent living
Qualifications
  • Current license as a Physical Therapist in Massachusetts
  • Minimum of one year of recent clinical experience
