Clinical Advanced Practice Provider

Longevity Health is expanding its team and is seeking a dedicated Clinical Advanced Practice Provider – Nurse Practitioner or Physician Assistant.
Position Summary: The Clinical Advanced Practice Provider, either a Nurse Practitioner or Physician Assistant, is a licensed and board-certified professional tasked with providing personalized care to members of Longevity Health. This role is crucial in executing the Institutional Special Needs Plan Model of Care, ensuring that each member receives tailored medical attention.
Core Responsibilities: The individual in this role is expected to uphold the mission of Longevity Health, which is to enhance the health and quality of life for nursing home residents by addressing their specific medical, social, and emotional needs.
The Clinical Advanced Practice Provider will:
- Manage a designated panel of nursing home residents, collaborating closely with the primary care team to implement the Longevity Health Model of Care.
- Conduct thorough assessments and create individualized care plans for each member.
- Participate in care team meetings and maintain open communication with residents' families to ensure alignment with their care goals.
- Utilize clinical guidelines to inform treatment decisions and maintain accurate documentation of health and functional status.
- Work alongside the Clinical Manager and local medical director to receive ongoing education and support in best practices.
Key Duties Include:
- Providing collaborative care to a panel of members, ensuring treatment aligns with their personal care goals.
- Implementing the Longevity Health Model of Care and advanced care planning.
- Managing clinical operations such as prior authorizations and quality gap closures.
- Acting as a trusted resource for members, families, and nursing home staff, advocating for member needs.
- Facilitating effective communication with primary care providers and care teams regarding treatment plans.
- Balancing caseload demands while supporting team members.
- Promoting Longevity Health's mission within the community.
- Functioning autonomously while being accountable for clinical practice.
- Building meaningful relationships within the nursing home environment.
- Ensuring a member-centric approach to care delivery.
- Assisting with additional duties as required.
Qualifications:
- Must hold a state unrestricted license as an Advanced Practice Nurse or be a board-certified Physician Assistant.
- DEA License is mandatory.
- Active national provider number (NPI) and Medicare provider number required.
- Valid driver's license and proof of automobile insurance are necessary.
Education:
- A Master's degree in Nursing or a related field from an accredited institution.
Experience:
- At least 6 months of experience in a clinical setting, preferably with geriatric or hospice care.
- Experience in a nursing home environment is highly desirable.
- Proficiency in technology and excellent interpersonal skills are essential.
