JOB SUMMARY

The DAC Peer Support Specialist will serve as a vital link between the community and the Department of Adult Corrections (DAC) re-entry program, providing support and coordination for individuals with serious mental illness (SMI) transitioning back into society. This role is responsible for ensuring seamless communication between DAC, local management entities/managed care organizations (LME/MCOs), and community behavioral and physical health providers.

KEY RESPONSIBILITIES

  • Case Management and Coordination: Develop and implement individualized care plans to address the unique needs of each participant, including medication management, housing support, and other social determinants of health.
  • Collaboration and Communication: Foster strong relationships with DAC staff, LME/MCOs, and community providers to ensure a coordinated approach to re-entry services.
  • Community Outreach and Engagement: Establish and maintain connections with community stakeholders, including natural supports, to promote a person-centered approach to care.
  • Documentation and Reporting: Maintain accurate and timely records of participant progress, service utilization, and outcomes.

QUALIFICATIONS AND REQUIREMENTS

  • A North Carolina Certified Peer Support Specialist (NCPSS) certification is required.
  • At least two years of experience providing similar services to populations served (MH/SU/IDD) is preferred.

WORK ENVIRONMENT

This is a mobile-based position that requires frequent travel within the community. The employee must reside in North Carolina or within 40 miles of the NC border. Vaya Health is an equal opportunity employer.
