We are seeking a skilled Mental Health Case Coordinator to join our team at Denton County MHMR. As a key member of our staff, you will play a crucial role in providing essential services to individuals served.

About the Job
  • This position involves developing and maintaining long-term contact with clients, families, and service providers for medical, social, educational, and related service needs.
  • You will oversee the care of individuals assigned to various Levels of Care (LOC), primarily LOC1, LOC2, and LOC3, ensuring continuity of care.
Responsibilities
  • Attend and participate in all scheduled team meetings, taking notes and reviewing information.
  • Provide courteous, professional services at all times, maintaining professional boundaries and interacting with Center staff, individuals, and community members in a respectful manner.
  • Deliver case management and psychosocial rehabilitation services, including supported employment, housing, budgeting, skills training, and other services as defined in Texas Resilience Recovery (TRR) guidelines.
  • Assist other teammates in their absence by providing services to individuals on their caseload as needed.
  • Complete community-based visits, provide transportation for individuals as needed, and ensure everyone served receives required services.
Requirements
  • Possession of a valid driver's license and current automobile insurance.
  • Ability to work independently, make decisions, and seek guidance when needed.
  • Knowledge of community resources, case management delivery systems, and program policies and procedures.
  • Skills in active listening, de-escalation techniques, empathy, and basic knowledge of social services systems.
