Responsibilities
As a Clinical Case Manager for Palliative Care, you will be responsible for the following key responsibilities:
  • Assessing patients' needs and developing individualized care plans
  • Coordinating with interdisciplinary teams to ensure seamless care delivery
  • Providing emotional support and guidance to patients and families
  • Communicating effectively with patients, families, and healthcare professionals


In addition to these responsibilities, you will also have the opportunity to participate in ongoing education and training, as well as contribute to quality improvement initiatives within the organization. Salary: $70,000 - $85,000 per year, depending on experience.
