Engagement Navigator

We are seeking a highly skilled Engagement Navigator to join our team at AcuteCare Health System LLC. As an Engagement Navigator, you will play a critical role in the successful intake and enrollment of potential PACE participants into our program.

• Build strong relationships with prospects, their families, caregivers, and other community stakeholders to effectively explain the PACE program, eligibility criteria, and enrollment processes.
• Schedule and facilitate assessments, ensuring effective communication with team members and adherence to person-centered processes.
• Track and analyze pertinent data and statistics to inform program development and improvement.

• Baccalaureate degree in healthcare administration, business, communications, marketing, or a related human services field.
• High School diploma or equivalent required; experience in community outreach, intake, enrollment, or marketing preferred.
• Reliable transportation, valid driver's license, and minimum state required liability auto insurance.
• Medically cleared for communicable diseases and up-to-date immunizations before engaging in direct participant contact.
• Pass a comprehensive criminal background check.
