Job Title: Engagement Navigator

We are seeking a highly motivated and compassionate Engagement Navigator to join our team at BoldAge PACE. As a key member of our outreach and engagement department, you will play a vital role in building strong relationships with potential participants, their families, and caregivers to promote our all-inclusive program of care.

Job Summary:

The Engagement Navigator is responsible for effectively explaining the PACE program, eligibility criteria, and enrollment processes to prospective participants and their support networks. You will work closely with the Director of Outreach and Engagement to develop and implement strategies to increase census growth and services to the community.

Key Responsibilities:
  • Build and maintain strong relationships with potential participants, their families, and caregivers to promote the PACE program.
  • Effectively communicate the program's benefits, eligibility criteria, and enrollment processes to prospective participants and their support networks.
  • Develop and implement strategies to increase census growth and services to the community.
  • Schedule and facilitate assessments, ensuring a person-centered approach to care.
  • Collaborate with team members to ensure effective communication and adherence to person-centered processes.
  • Track and analyze pertinent data and statistics to inform outreach and engagement strategies.
Requirements:
  • Baccalaureate degree in healthcare administration, business, communications, marketing, or a related human services field.
  • High school diploma or equivalent required.
  • Experience in community outreach, intake, enrollment, or marketing preferred.
  • Reliable transportation, valid driver's license, and minimum state required liability auto insurance.
  • Medically cleared for communicable diseases and up-to-date immunizations.
  • Pass a comprehensive criminal background check.
Skills and Abilities:

  • Excellent communication and interpersonal skills.
  • Commitment to promoting quality services to a frail client population.
  • Ability to be creative and innovative in developing systems and making decisions.
  • Understanding of diversity and inclusion principles.
  • Ability to maintain accurate records and prepare clear reports.
  • Excellent verbal and written communication skills.
  • Public speaking skills for large and small groups.
