Position Summary:

Under the guidance of the Director of Outreach and Engagement, the Engagement Navigator is responsible for the successful intake and enrollment of potential PACE participants into the program by building strong relationships with prospects, their families, caregivers, and other community stakeholders. The Navigator is responsible for effectively explaining the PACE program, eligibility criteria, and enrollment processes, and helping navigate through any barriers that may arise. Other duties include scheduling and facilitating assessments, effective communication with team members, adherence to person-centered processes, and tracking and analyzing pertinent data and statistics.

REQUIREMENTS

  1. A baccalaureate degree in health care administration, business, communications, marketing, or related human services field is preferred.
  2. High School diploma or equivalent is required.
  3. Experience in community outreach, intake, enrollment, or marketing preferred.
  4. Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  5. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  6. Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.

SKILLS AND ABILITIES

  1. A “people first” approach to all decision making, planning and program practices.
  2. Communication expertise with staff, colleagues, and clients in a variety of settings, including the ability to communicate clearly and to listen well.
  3. Demonstrated commitment to promoting quality services to a frail client population with a focus on continuous improvement and a “do it better” approach.
  4. Ability to be creative and innovative in developing systems and making decisions.
  5. Understanding of the principles of diversity and inclusion and ensuring their incorporation into the culture, policies, and practices of the outreach and engagement department.
  6. Ability to maintain a consistent and clear adherence to established, person-centered processes to promote continued census growth and services to the community.
  7. Skills to effectively represent the program to participants, families, outside agencies, community groups, community professionals and the general public.
  8. Effective relationship-building skills with potential enrollees, government agencies, community members, referral sources and co-workers.
  9. Knowledge of federal, state, and local regulations and requirements related to PACE outreach and engagement practices.
  10. Ability to maintain accurate records and to prepare clear and concise reports, correspondence, and other written materials.
  11. Excellent verbal and written communication skills.
  12. Public speaking skills for large and small groups.
  13. Familiarity with management information systems and electronic health records
