Job Summary

We are seeking a skilled LPN to join our Home Health team in the Far Western Suburbs. As a LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to achieve optimal health outcomes.

Responsibilities
  • Assure quality of care by facilitating patient and family rights, working under the direction of the RN, and adapting practice to the latest standards.
  • Utilize resources to respond to situations that may negatively impact patient outcomes, and participate in efforts to reduce risk and improve patient safety.
  • Document patient care thoroughly and decisively to support reimbursement and regulatory requirements.
  • Practice as an effective team member to formulate an integrated approach to care, recognizing changes in clinical situations and consulting with the RN as needed.
  • Make sound clinical decisions, prioritize patient care, and ensure clear hand-off communication to achieve optimal health outcomes.
  • Participate in interdisciplinary case conferences and seek opportunities to enhance knowledge, skill, and competency.
  • Develop and maintain a therapeutic relationship throughout the healthcare continuum, promoting patient/family knowledge and confidence to safely manage self-care.
  • Provide care that exceeds patient and family expectations, as measured by survey instruments.
Requirements
  • Licensed in the State of Illinois (or eligible for licensure) with 2 years of Med/Surgical clinical experience.
  • Certificate in Pharmacology and CPR certification.
  • Basic knowledge of computer use, including Microsoft Word and email access.
  • Ability to learn and use computer-based scheduling and documentation systems.
  • Strong communication and documentation skills, with good interpersonal skills and time management abilities.
Working Conditions

This position requires extensive travel within geographical boundaries, including all extremes of weather and on all types of roads. The LPN must be able to provide/arrange own transportation to and from patients' homes and branch offices. Ability to work rotating weekends and holidays, with adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned.

Compensation

We offer a competitive hourly rate of $24.35-$36.55, with a comprehensive benefits package including a sign-on bonus, competitive pay, referral bonus programs, tuition assistance, and mileage reimbursement.
