About Us

Responsibilities
  • Evaluate patients' functional abilities and develop tailored treatment plans to address their specific needs
  • Implement and supervise physical therapy programs, ensuring effective and efficient delivery of services
  • Communicate regularly with patients, families, and caregivers to provide updates on progress and address concerns
  • Collaborate with multidisciplinary teams to ensure seamless delivery of care
  • Document patient information, treatment plans, and progress in accordance with regulatory requirements
Requirements
  • Bachelor's degree in Physical Therapy required; Illinois licensure in Physical Therapy required within 6 months of hire
  • Must obtain CPR certification within 30 days of hire and maintain throughout employment at CGH Medical Center
  • Valid Illinois Vehicle Operator's License required
