About the Role

Hammonton Center for Rehabilitation and Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a dynamic and supportive environment. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Provide Advice and Support: Offer guidance and support to residents and their families, addressing their needs and concerns in a compassionate and empathetic manner.
  • Monitor Residents and Administer Medication: Conduct regular assessments of residents' health and administer medication and treatments as prescribed by our medical team.
  • Document Resident Care Services: Accurately record resident care services in our electronic medical records, ensuring that all necessary information is up-to-date and easily accessible.
  • Protect Residents and Staff: Adhere to our infection-control policies and protocols, maintaining a safe and healthy environment for all residents and staff.
  • Resolve Resident Problems and Needs: Utilize our multidisciplinary team strategies to address resident concerns and needs, ensuring that each individual receives the highest level of care.
  • Assure Quality of Care: Adhere to our facility's philosophies and standards of care, maintaining the highest level of quality and excellence in all aspects of resident care.
  • Maintain Resident Confidence: Ensure the confidentiality of resident information, maintaining trust and respect in all interactions.
Requirements
  • Valid LPN License: Possess a current and valid LPN license, demonstrating your commitment to providing high-quality care.
  • Experience with Point Click Care: Preferably have experience with Point Click Care, a leading electronic medical record system.
  • Basic Computer Skills: Possess basic computer skills, including proficiency in electronic medical records and other healthcare software.
  • Excellent Communication Skills: Demonstrate excellent communication skills, both written and verbal, ensuring effective collaboration with our team.
