Key Responsibilities:- Wound Debridement and Management: Safely perform wound debridement using sharp, enzymatic, autolytic, or surgical methods at the patient's location.
- Topical Wound Care and Product Management: Apply and manage topical wound care and related wound management products.
- Diagnostic Testing and Interpretation: Interpret diagnostic tests and suggest suitable follow-ups.
- Prescription and Referral: Prescribe necessary medications, Durable Medical Equipment (DME), or supplies, aligned with the supervising physician's collaborative agreement.
- Risk Assessment and Education: Conduct risk assessments for Diabetic Foot Ulcers in diabetic patients as per ADA guidelines and educate patients about wound management.
- Collaboration and Communication: Collaborate with internal and external medical partners to ensure comprehensive care and treatment of in-home and facility-based patients.
- Education and Licensure: Successful completion of an accredited nurse practitioner program or licensed physician assistant.
- State Authorization and License: State-approved Board of Nursing authorization to perform medical tasks and active, unrestricted state APRN license.
- Skills and Abilities: Exceptional verbal/written communication, strong interpersonal and organizational skills, dedicated to excellence and diligence in daily workflow, strong antilytic and problem-solving skills, and task prioritization mastery.
- Work Environment: Potential exposure to indoor/outdoor environmental challenges, blood borne pathogens, physical (e.g., needle pricks) and chemical hazards.
- Physical Demands: Ability to stand/walk for over 4 hours, ability to lift up to 100 pounds for resident assistance, regularly lift 10 pounds, bending, squatting, kneeling, twisting during patient care.
- Salary: $65.00 per hour.

- Job Type: Full-time, contract.
- Schedule: Monday to Friday with no weekends.
