About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at LHC Group as a Full-Time Home Health Nurse. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities

  • Provide high-quality patient care and perform technical skilled care in the patient's home, adhering to the plan of care and in coordination with other members of the healthcare team.
  • Continually observe and report the patient's nursing needs to the supervising RN, ensuring seamless care delivery.
  • Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs, visit assignments, and schedule changes.
  • Document patient visits thoroughly and completely, maintaining accurate and current medication listings, and adhering to policy and payer requirements.
  • Actively participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.

Requirements

  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
